Sometimes the most profound moments of care happen far outside the walls of a hospital. They happen in community centers, parking lots, and mobile units — wherever people are, wherever need exists. That is the heart of our mission at Ascension St. Vincent's Foundation, and it is brought to life every day through programs like Medical Mission at Home and our Mobile Health Outreach Ministry.
The story you are about to read is one we are honored to share. She asked that we protect her privacy, so we'll call her Carol — but her story, and her words, are entirely her own.
When Carol moved from California to Nassau County Florida to be closer to family, she knew she was bringing more than just boxes and belongings. With less than seven months remaining before becoming eligible for Medicare, she also carried the weight of having no insurance while living with multiple chronic health conditions, including a prior stroke (CVA), pulmonary concerns, neurological challenges, and an injury that had significantly impacted her ability to work.
Navigating healthcare in a new community can be overwhelming, especially when medical needs are complex and complicated by the fact you are without insurance. Fortunately, her daughter saw a special event happening she thought would be of benefit to her mom. Through Medical Mission at Home, she found a coordinated approach to care that addressed her concerns and put her on a path to established and consistent healthcare. From the very beginning of her visit, she was navigated through the day and received a comprehensive assessment that included an EKG and referrals to specialists who could address her immediate needs.
Carol’s care journey at the Medical Mission at Home event quickly expanded beyond a single point of contact with a primary care provider. She was connected with a pulmonologist to evaluate her respiratory concerns and a neurologist to address ongoing complications related to her previous work injury that had not been resolved. The neurologist referred her to physical therapy, where she received treatment the very same day and was equipped with exercises she could continue at home to support her recovery and independence.
But her story did not end there.
Recognizing that healthcare is not a one-time event but an ongoing relationship, Ascension St. Vincent’s Mobile Health Outreach Ministry ensured she remained connected to services through our mobile unit, which travels to Nassau County each month. She was scheduled to establish care with our mobile primary care provider, creating a medical home where her chronic conditions could be monitored and managed. Through this continued care, she has now been provided with essential medications and education as well as laboratory services that provide critical information for guiding treatment decisions and supporting her long-term health.
What stood out most to her was not simply the services she received, but the seamless continuum of care that followed her every step of the way—from Medical Mission at Home to the Mobile Health Outreach Ministry program and ongoing mobile primary care services. Her healthcare needs are being met by bringing vital healthcare services to her in the community in which she lives, at no cost to her thanks to the generosity of our donors.
Reflecting on her experience, Carol shared, “This is the best thing I’ve ever done in my life. This care is unlike anything I’ve ever experienced before.”
Her story is a reminder how we can step outside the traditional clinical setting to accomplish our mission of bringing compassionate care to people where they live.
